This will be an in-office role in Calhoun, GA.

Arkos Health is seeking a Care Navigator. The Patient Care Navigator will be responsible for ensuring seamless care delivery, by optimizing teamwork flows, managing essential processes, and maintaining precise documentation. They will assist the care team in navigating healthcare services, identifying barriers to care and collaborating with clinical team to enhance patient experiences and outcomes. The Patient Care Navigator will directly support the team's ability to reach patient care goals, reduce avoidable hospitalizations, and improve HEDIS improvement measures.

Essential Duties and Responsibilities:

  • Act as a liaison between providers, patients, and Interdisciplinary Care Team to enhance care continuity
  • Prepare pre-visit checklist for patient appointments and flag urgent cases for review if needed
  • Proactively identify gaps; flagging missing preventive services/screens using the pre-visit workflow and coordinate needed equipment for clinical team
  • Review member utilization for patient appointments (ED, Admits, etc.) for pre-visit checklist
  • Extract Medical Diagnoses Form (MDF) for providers and coordinate with risk department, if needed, for updated MDF information and attach to patients chart
  • Update huddle bord for next day’s regional huddle strategy session and upload to SharePoint for record keeping Patient Navigator_03.28.2025 1
  • Participate in huddles, team meetings, patient case discussions, and quality improvement initiatives
  • Maintain appointment optimization by identifying telehealth visits, and coordinating Quality Coordinators to complete in home visits to close gaps
  • Support team members by optimizing appointment management to prevent loss of patient follow up
  • Monitor real time hospital, SNF, and emergency department alerts from Arkos 360, Experian, and HIE ensuring timely follow up
  • Assist corporate call center coordinators to ensure filed orders and requests are processed timely
  • Obtain requested medical records in a timely manner
  • Upload all orders, field documents, and requested records to the patient’s charts
  • Demonstrate excellence and accuracy in collection of information, knowledge of processes, and be committed to providing outstanding delivery of patient care
  • Maintain accurate and confidential patient records and documentation related to care delivery
  • Maintain active patient panels in EHR and manage time sensitive requests including but not limited to lab results and prescription renewals
  • Ensure field team documentation aligns with risk and HEDIS expectations for gap compliance
  • Perform other duties as assigned to support the overall effectiveness of the organization

Required Skills & Qualifications:

  • High school diploma, GED, or equivalent work experience
  • Some medical office experience
  • Knowledge of medical terminology and patient care
  • Language & Communication Skills: excellent leadership, management, and communication skills with ability to work collaboratively with other departments and external stakeholders; must be fluent in both spoken and written English and maintain professionalism in all verbal and written correspondence with both internal and external clients.
  • Decision Making: strong ability to self-manage time to manage multiple projects and priorities in a fast-paced environment
  • Computer Skills: able to utilize necessary programs to ensure effective communication, collection of metrics, and proprietary software; including but not limited to: Microsoft product suite, Adobe Suite
  • Problem Solving: strong analytical and problem-solving skills

Preferred Skills & Qualifications:

  • Some college
  • MA, CNA, or medical certification
  • Knowledge of medications, health plans, health insurance, or medical background and terminology
