How You'll Make a Difference:

The Healthcare Outreach Specialist is the frontline of patient engagement, responsible for the prompt scheduling of patient appointments, fostering goodwill between the health plan, our organization, providers and patients. The position is responsible for managing and coordinating the needs of multiple parties, applicable appointments in an efficient manner while accommodating the needs of members.

Essential Duties and Responsibilities:

  • Under the guidance of the Patient Outreach Supervisor, the specialist collaborates with interdepartmental team members to coordinate all levels of scheduling from basic to complex
  • Accurately and efficiently coordinates and tracks scheduling of visits to be conducted by Nurse Practitioners, quality coordinators, RN Care Managers, or other providers as assigned
  • Conducts telephone calls to members, providers, and other health care professionals and organizations in support of care initiatives
  • Receives, proofreads, and verifies reporting from the health plan against internal systems for accuracy and completeness to appropriately schedule appointments
  • Revises schedules as cancellations and/or additions occur and clearly and timely communicates with the appropriate personnel
  • Conducts patient visit reminder calls in advance of scheduled “In Home” appointment upon request.
  • Process requests for member medical records if applicable and/ and or upon request
  • Scan/Enter/Upload member medical records and documentation into appropriate electronic repository to align with member record
  • Accurately and efficiently coordinates and tracks scheduling of member referrals and visits to be conducted by Nurse Practitioners
  • Ability to utilize multiple systems simultaneously to schedule member visits

Qualifications and Skills:

  • High school diploma or GED equivalent required
  • Completion of 1-2 years post-secondary education preferred. Certification requirement (if applicable)
  • 2+ years' experience in a healthcare environment preferred
  • CNA / RMA / CMA preferred
  • Excellent interpersonal communication (written and verbal) skills
  • Experience with Electronic Medical Record
  • Knowledge and use of Microsoft Office applications preferred
